Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. Background
Pragmatic trials provide real-world evidence that can be used to make clinical decisions. This is especially important in trials that involve surgical procedures that are invasive or have potentially serious adverse events. The CRASH trial29, for example was focused on functional outcomes to compare a 2-page case-report with an electronic system for the monitoring of patients admitted to hospitals with chronic heart failure. Similarly, the catheter trial28 focused on symptomatic catheter-associated urinary tract infections as its primary outcome.
In addition to these aspects, pragmatic trials should minimize the trial procedures and requirements for data collection to reduce costs. In the end these trials should strive to make their results as applicable to current clinical practice as is possible. This can be achieved by ensuring their primary analysis is based on the intention to treat approach (as defined in CONSORT extensions).
Despite these requirements, many RCTs with features that defy the concept of pragmatism have been mislabeled as pragmatic and published in journals of all types. This can lead to false claims of pragmatism, and the usage of the term needs to be standardized. The creation of the PRECIS-2 tool, which provides an objective standard for assessing pragmatic characteristics, is a good first step.
Methods
In a practical trial it is the intention to inform clinical or policy decisions by demonstrating how an intervention would be implemented into routine care. Explanatory trials test hypotheses concerning the cause-effect relation within idealized conditions. In this way, pragmatic trials could have a lower internal validity than explanation studies and are more susceptible to biases in their design as well as analysis and conduct. Despite their limitations, pragmatic research can be a valuable source of data for making decisions within the healthcare context.
The PRECIS-2 tool scores an RCT on 9 domains, with scores ranging between 1 and 5 (very pragmatist). However, pragmatic trials can also have drawbacks. The right amount of heterogeneity, like could allow a study to extend its findings to different settings or patients. Numerous studies have attempted to classify pragmatic trials with various definitions and scoring systems. It is important to remember that the term “pragmatic trial” does not necessarily mean a low-quality trial, and in fact there is an increasing number of clinical trials (as defined by MEDLINE search, however this is not specific or sensitive) that employ the term “pragmatic” in their abstracts or titles. The use of these terms in abstracts and titles may suggest a greater awareness of the importance of pragmatism but it is unclear whether this is evident in the content of the articles.
Conclusions
As the value of evidence from the real world becomes more commonplace and pragmatic trials have gained momentum in research. They are randomized trials that evaluate real-world care alternatives to experimental treatments in development. They are conducted with populations of patients closer to those treated in regular medical care. This approach can help overcome the limitations of observational research which include the biases that arise from relying on volunteers and limited availability and the variability of coding in national registries.
Pragmatic trials also have advantages, such as the ability to draw on existing data sources, and a greater probability of detecting meaningful distinctions from traditional trials. However, these tests could still have limitations which undermine their effectiveness and generalizability. The participation rates in certain trials may be lower than expected because of the healthy-volunteering effect, financial incentives or competition from other research studies. The need to recruit individuals quickly restricts the sample size and impact of many pragmatic trials. Additionally certain pragmatic trials lack controls to ensure that the observed differences are not due to biases in trial conduct.
The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-labeled themselves as pragmatist and published from 2022. The PRECIS-2 tool was used to evaluate pragmatism. It includes domains such as eligibility criteria, recruitment flexibility and adherence to intervention and follow-up. They discovered that 14 of these trials scored as highly or pragmatic pragmatic (i.e. scoring 5 or higher) in one or more of these domains, and that the majority of these were single-center.
Studies with high pragmatism scores are likely to have more lenient criteria for eligibility than conventional RCTs. They also have patients from a variety of hospitals. These characteristics, according to the authors, may make pragmatic trials more useful and applicable in the daily clinical. However, they don’t guarantee that a trial is free of bias. Furthermore, the pragmatism of the trial is not a definite characteristic and a pragmatic trial that does not have all the characteristics of a explanatory trial may yield reliable and relevant results.
